He was essentially flipping the bird at his critics. They mauled his first symphony, and he became very depressed, but the Piano Concerto No. 2 was his way of recovering and restoring his self-confidence – it was dedicated to his physician.

Classical music is not usually 'based on' anything – it is 'absolute' music written for its own sake. Alex above is correct that this was the first major piece that Rakhmaninov wrote after the catastrophic failure of the first performance of his First Symphony in 1897. It is true that, as a gesture of gratitude for restoring his confidence back to a level where he though he could compose again, Rakhmaninov dedicated the piece to his psychiatrist Nikolai Dahl. However, it would be a mistake to think that this was any kind of 'story' behind the music.
