Ivan IV was the builder of modern Russia. He made himself the first Czar, and greatly reduced the aristocracy that corrupted the rule of government. He was a loving and dedicated ruler who ruled absolutely.
Following the ideals of his father Ivan III he made all nobles, ministers, merchants, and farmers servants of the Czar. He administered his rule through a system called the Oprichnina. If you got on the wrong side of Ivan, it is the secret police of the Oprichnina that you would answer to. From an historical perspective Ivan's achievements make him either the 1st or 2nd greatest ruler in Russian History. From the liberal ideal of finger pointing, he was a mean man who hurt people for absolutely no reason.

For the most part, "terrible" really meant "strict, firm, determined, and utterly powerful." Ivan did have some positive effects because of his strictness.
But he also committed some terrible outrages, including — quite famously — murdering his own son. See below:
