Question by With her head in the clouds: Why didn’t Tsar Nicholas 2nd and Tsarina Alexandra try for another son after Alexei?
They would have been aware that Alexei wasn’t likely to survive to be Tsar and it would be unlikely he’d survive long enough to have an heir. (He wouldn’t anyway) but why didn’t they they try for a second hopefully healthier son who could take the throne if Alexei didn’t survive to take it.

They had four healthy girls before Alexei. Odds are they DID try, but no one outside the royal household knew whether or not they did.

Of course they tried that. Every royal family always goes for ‘an heir and a spare’.
Even if there is an heir, one never knows what will happen to him in the future.
To give you an example of the latter: King William III of The Netherlands did have not one but three healthy sons. However, all his children died before him of natural causes. When the last one died, William III was already an (very) old man. He remarried and was still able to produce a daughter who became later on queen. (Wilhelmina)

They did. There were two miscarriages reported after Alexei. But she wasn’t in vigorous health.
